| Chem 190/199 Form | For undergraduates students who have already spoken with their PI. |
| Chem 190/199 (Less than 3.0) Form | For undergraduates students with less than a 3.0 GPA who have already spoken with their PI. |
| Key Request Form | Keys and access to the research labs are requested by the faculty. |
| Safety Quiz | The Safety Quiz will be assigned to you via Canvas every semester, so watch for the emails. |
| Chem 290/295/299c Form | For graduate students who have already spoken with their PI. |
| Chem 290/295/299c (Less than 3.0) Form | For graduate students who have already spoken with their PI. |
| Chem 299 | Form for the graduate student's thesis committee is available on the Graduate Studies Forms page, from the Division of Research and Graduate Studies. |
| Chemistry & Biochemistry Volunteer Form | Volunteer Forms are required for anyone volunteering in a research lab who are not being paid and not registered as a student enrolled in classes at Fresno State. |
